Create makes a new Conversation from the inputs below. It is inherited: every Noun has get, list, create, update and delete.

`resolution`	`string`		How it was resolved: the Customer said so, or 72 hours passed after an answer with no reply.

`customer`	Customer		The Customer a Conversation serves, so every answer reads that Customer’s records.
`issue`	Issue		The Issue or Incident a Conversation reports; resolving it answers every linked Conversation.

Delay window	None: it runs once review passes.
Limit	The call Limit of the Permission it runs under.
Over the Limit	It waits `pending` for an Approval from the Founder. Approved, it runs; refused, it ends `refused` with `limit-exceeded`; unanswered by its deadline, it ends `canceled`.
